Otter delivered his State of the State and budget address on Monday.<\/p><\/div>\n<p>In Monday&#8217;s <a title=\"Gov. Otter\u2019s State Of The State Speech Sets His Agenda For Idaho Legislature\" href=\"http:\/\/stateimpact.npr.org\/idaho\/2013\/01\/07\/gov-otters-state-of-the-state-speech-sets-his-agenda-for-idaho-legislature\/\">State of the State address<\/a>, Gov. C.L. &#8220;Butch&#8221; Otter made clear to lawmakers and the public that he&#8217;s not pushing to <a title=\"An Essential Guide to Idaho\u2019s Medicaid Program\" href=\"http:\/\/stateimpact.npr.org\/idaho\/topic\/medicaid\/\" target=\"_blank\">expand Medicaid eligibility<\/a> at this time.<\/p>\n<p>Instead, he has asked Idaho Department of Health and Welfare Director Richard Armstrong to develop an Idaho-specific plan.<\/p>\n<p>As <em>StateImpact<\/em> <a title=\"What Gov. Otter Meant When He Said \u201cNo\u201d To Expanding Medicaid Eligibility\" href=\"http:\/\/stateimpact.npr.org\/idaho\/2013\/01\/09\/what-gov-otter-meant-when-he-said-no-to-expanding-medicaid-eligibility\/\">reported yesterday<\/a>, Armstrong says the federal Centers for Medicare and Medicaid Services (CMS) will have to sign off on any effort by the state to build cost-sharing and accountability mechanisms into its plan.\u00a0 On this point, there&#8217;s a great deal of uncertainty.\u00a0 <!--more--><\/p>\n<p>Last month, CMS released <a href=\"http:\/\/cciio.cms.gov\/resources\/files\/exchanges-faqs-12-10-2012.pdf\" target=\"_blank\">answers<\/a> to some of the many questions it has received from states regarding implementation of the Affordable Care Act.\u00a0 In that document, CMS says it will support options promoting personal responsibility for the Medicaid expansion population &#8220;depending on its design.&#8221;<\/p>\n<div class=\"related-content alignleft\"><h4 class=\"related-header\">Related<\/h4><div class=\"links\"><h5>Posts<\/h5><ul><li class=\"link\"><a href=\"http:\/\/stateimpact.npr.org\/idaho\/2013\/01\/09\/what-gov-otter-meant-when-he-said-no-to-expanding-medicaid-eligibility\/\">What Gov. Otter&#8217;s State Of The State Speech Sets His Agenda For Idaho Legislature<\/a><\/li><li class=\"link\"><a href=\"http:\/\/stateimpact.npr.org\/idaho\/2012\/11\/09\/idaho-should-expand-medicaid-eligibility\/\">It&#8217;s Unanimous: Idaho Should Expand Medicaid Eligibility<\/a><\/li><\/ul><\/div><div class=\"topics\"><h5>Topics<\/h5><p class=\"topic\"><img class=\"alignleft\" src=\"http:\/\/stateimpact.npr.org\/idaho\/files\/2011\/12\/12-9-Medicaid-60x60.jpg\" height=\"60\" width=\"60\" \/><a href=\"http:\/\/stateimpact.npr.org\/idaho\/topic\/medicaid\/\">An Essential Guide to Idaho&#8217;s Medicaid Program<\/a><\/p><\/div><\/div>\n<p>Robin Rudowitz of the Kaiser Commission on Medicaid and the Uninsured says more explicit federal guidance is still to come.\u00a0 <del><\/del>It&#8217;s not yet clear how receptive the federal government will be to states&#8217; requests to include cost-sharing requirements (like monthly premiums or co-pays) in the coverage they extend to the expansion population.<\/p>\n<p>Rudowitz says it&#8217;s hard to know how willing the federal government will be to waive federal Medicaid rules and allow states flexibility in structuring their plans.\u00a0 \u201cI think there are two forces that will be at work: states that want waivers, and the federal government that wants them to move ahead with the expansion,\u201d she says.<\/p>\n<p>The federal government will initially cover 100 percent of the cost of expanding Medicaid eligibility to 138 percent of the federal poverty level.\u00a0 <a href=\"http:\/\/healthandwelfare.idaho.gov\/Portals\/0\/Medical\/MedicaidCHIP\/Milliman%20preliminary%20draft.pdf\" target=\"_blank\">According to consulting group Milliman<\/a>, the optional Medicaid expansion would extend health care coverage to about 80,000 low-income adults in Idaho.\u00a0 Milliman also estimates the expansion would save the state and counties $6 million over 10 years.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>In Monday&#8217;s State of the State address, Gov.
